Hazelwell Lodge is an award-winning
Residential Care Home for
the elderly situated in the parish town
of Ilminster in South West Somerset. We are
a family run business since 1987, for people
with old age and dementia, with an aim to change
people’s perception of care homes and how people
view their autumn years.

We are Specialists in care for those who live with
dementia and associated behavioural issues that may
cause challenges in an unsupervised setting Hazelwell
enables such residents to resume an appropriate
and desirable way of living that deals with their
individual needs on a personal level. We provide
safe environments that are adapted to the
unique needs of the resident so that they can enjoy as much independence, mental and physical stimulation as possible.

We create a personalised care plan which is regularly updated to ensure individual needs are constantly met. This involves discovering details about a resident’s lifestyle and medical history, preferred activities and interests, hobbies and dietary and food preferences.

With the support and interaction of local authorities, NHS and Healthcare professionals it is clear to see that perceptions are changing, and Hazelwell Lodge holds an important and respected position in the local community.

Training

Training

At Hazelwell we have a strong focus on continued staff training and development. Our staff are trained to the National Standards upon joining the company and all workers complete a comprehensive induction programme which includes manual handling, infection control, first aid, Protection of vulnerable adults, fire safety and medication training In addition, we have developed our own in-house training courses based upon the unique working style of Hazelwell Lodge.

Claire Kingdon the home manager is responsible for the overall running of the home and is Passionate about ensuring that all care given to residents is maintained to the highest possible standards at all time. Claire has 22 years’ experience in the care sector 20 of which have been specialising in Dementia Care. Claire has achieved the N.V.Q level 4 in Health & Social care alongside the Registered Managers Award qualification. She is a qualified Dementia care lead and a Health and Social Care N.V.Q diploma Assessor.

Claire is supported by her deputy Michelle Trott and Heads of Care, Sophie Gummer and Lisa Ward. Their primary role is to develop and oversee the care management in the best interest of the residents, families and carers. All have obtained or are working towards NVQ diploma level 5 Health and Social care. The Deputy and heads of care are trained to act on behalf of the manager in her absence.

To achieve our objective of providing safe, high quality care, our staff are knowledgeable, skilled, compassionate Care Workers who understand your needs and provide personalised care. All our staff are fully trained to Care Quality Commission standards or higher, DBS checked for your safety and security and highly motivated carers as they know they are highly valued and acknowledged by us.

A shift leader is designated to each shift and is qualified to take charge of the Home when the manager/head of care is off duty. All Shift leaders have obtained or are working towards the N.V.Q level 3 Health & Social Care Award. A senior manager is contactable at all times.

98% of our care related staff are working towards or have already obtained an NVQ/Diploma 2/3 in Health & Social Care.

We feel it is the Hazelwell people that make us one of the most highly regarded care providers in the area. Our team of skilled and passionate staff are dedicated to providing the highest standard of service 24 hours a day. They are always approachable, and are respected not only by our residents, but also by their families and medical professionals.

Support Staff

Our ancillary staff provide support services to the carers and activity staff. This includes all housekeeping and domestic services, including washing and ironing, regular and thorough cleaning of all areas. The chef and kitchen staff provide nutritious, delicious home cooked food for our residents always paying particular regard to specialist needs such as Diabetes and difficulties in swallowing. The menus are regularly changed according to seasonality and individual preferences and the food is sourced locally wherever possible. Our maintenance team apart from keeping the high standards in the home are always willing to interact with our residents in particularly activities involving gardening.

The Care Plan ensures that we can deliver high quality, person-centred care.

Once the assessment is completed we, together with you, will develop a care plan which shows us the best way to provide services to meet your identified needs, using your preferred approach. It will detail the support that you need and the help or services you will get from us. The care plan will be reviewed together on a monthly basis or as and when changes occur.

Personalised care planning empowers individuals, promotes independence and helps people to be more involved in decisions about their care. It centres on listening to individuals, finding out what matters to them and finding out what support they need.

A Holistic Approach

Personalised care planning is essentially about addressing an individual’s full range of needs, taking into account their health, personal, family, social, economic, educational, mental health, ethnic and cultural background and circumstances. It recognises that there are other issues, in addition to medical needs, that affect a person’s total health and well-being. It is therefore a holistic process, treating the person “as a whole” with a strong focus on helping people, together with their carers, to achieve the outcomes they want for themselves. In doing this, it should open up more choices that are relevant for people with long term conditions.
